You will need to fill out an “Application For The Issuance Of A Special License Plate And Affidavit Of Need And Eligibility.” The application requires that a co-owner of the vehicle or a member of your family must depend on the vehicle for the “necessities of life and would be subject to undue hardship” without the special license plate. The necessities of life include needing a vehicle to access food, water, shelter, and healthcare.

The application also requires proof that you surrendered your license plate, proof of vehicle insurance, and the family member or co-owner’s driver information, including their full name, date of birth, driver’s license number, address, and phone number.

If your hardship license plate application was denied, you can submit a request for an administrative hearing in front of the Office of the State Administrative hearings by submitting a written request to the Department.
